Admission Information

There are 3 steps to apply:

1. First:

Apply to Polk State College as a dual enrollment student for Fall 2025.

Allow 3-5 days to receive your Welcome Email. Polk State College Application

2. Second:

After receiving the Polk State Welcome email and Student ID number schedule to take the PERT Exam.

Schedule your PERT at Chain of Lakes Collegiate High School.

3. Third and last:

Apply to Chain of Lakes Collegiate High School using your Polk State College email.

Ensure you have the required documents (test scores, high school course history/transcript) to complete the application.

Make sure to complete these steps before the enrollment deadline on March 7, 2025, for consideration at Polk State Chain of Lakes Collegiate High School

Requirements for Admission

Incoming 10th Grade Applicants (Current 9th Graders):
  • Minimum of 5 high school credits at the end of your ninth-grade year
  • Unweighted GPA of 3.0 or higher
  • Earn the following PERT scores:
    • Math – 100
    • Writing – 92
    • Reading – 92
Incoming 11th Grade Applicants (Current 10th Graders):
  • Minimum of 11 high school credits at the end of your tenth-grade year
  • Unweighted GPA of 3.0 or higher
  • Earn the following PERT scores:
    • Math – 114
    • Writing – 103
    • Reading – 106

Students may apply during their Freshman or Sophomore year of high school. Students who have not yet completed an application to Polk State College may do so at http://passport.2fitfashion.com/Genesis/student/applogon.jsp. Students will need PERT scores to apply to Polk State Chain of Lakes Collegiate High School.
